Shifted Haemostatic Balance in Patients with Recurrent Venous or Arteral Thrombosis

In patients with recurrent idiopathic venous thrombosis (100) or occlusive arterial disease (50) 15 different variables of the haemostatic system have besn measured several weeks after the acute thrombotic event. In the cases with venous occlusion elevated F VIII coagulant activity was found in 52%, reduced fibrinolytic potential (euglobulinclot lysis time after provocation) in 53%, low antithrombin III activity in 31% and an increased number of circulating platelet aggregates in 24%. The corresponding figures for the arterial cases are 29%, 50%, 21% and 42% respectively. Only 13% of the venous and 21% of the arterial cases had no abnormality. Two or more abnormal variables were combined in 60% of all patients. All combinations of abnormal values were possible. These data show persistent changes of the haemostatic balance in moat patients with venous or arterial thrombotic disease. Laboratory investigation of patients with clinically demonstrated thrombotic tendency discloses the presence of alterations in coagulation, fibrinolysis, platelet reactivity and coagulation inhibitors and leads to the choice of either oral anticoagulants or fibrinolytic stimulants or platelet-active drugs.
