Trends in Prevalence of Esophageal Adenocarcinoma: Findings from a Statewide Database of over Six Million Patients
Purpose The rising incidence of esophageal adenocarcinoma (EAC) has plateaued in recent years. However, the temporal trends in the prevalence of EAC have not been well described. This study aimed to assess the prevalence of EAC based on age groups in a large database. Design This study was conducted using electronic health records from the OneFlorida Clinical Data Research Network. We used ICD codes to identify the disease conditions from 2012 to 2019. The primary outcome of interest was the adjusted prevalence of EAC. Age was categorized into three groups: young (18–44 years), middle-aged (45–64 years), and elderly (>65 years). Regression analysis was used to assess disease trends over time. Results The database included 6,872,194 adult patients. In the middle-aged group, the prevalence of EAC increased from 14.6 to 24.1 per100,000. The highest rate of increase was in the 61-70 years age group (y = 4x – 7496). An increasing prevalence was noted across the sex groups and all race-ethnicity categories. The prevalence of BE and GERD also increased in the same age group. The rate of increase in BE prevalence was the highest in the 51-60 years age group (y = 73.5x – 147494). The incidence rate of EAC was stable in this age group. Conclusions In this large study of healthcare databases, the prevalence of EAC and BE was found to be increasing in middle-aged patients. These trends do not seem to be related to the increased use of endoscopy as a screening test.
