Heparin Usage in a Community Hospital
Because of the immediate risks and serious later sequelae of thromboembolism, effective prophylactic treatment is warranted. Most patients on heparin therapy have no underlying hemostatic defect so that heparin therapy is relatively safe when given by different methods but the reported incidence of bleeding in heparinized patients vary from 8 to 30 percent. Few prospective trials relating to heparin administration have been published. The vagaries of laboratory monitoring were evaluated in 100 consecutive hospitalized patients; the thrombin clot time was felt to be the rvost practical method. Adverse reactions to heparin include immunothronbocytopenia, 2 cases were evaluated. Bleedinq of a major or minor degree occurred in 5% of patient. sFor the patient with venous thrombosis, continuous intravenous infusion of heparin for 7 to 10 days with monitoring by the thrombin clot time is advised at a dose level of 400 to 6O0 U/Kg/24 hours.
