HeParin Rebound After Extracorporeal Circulation in Cardiac Surgery: Experimental Study.

The heparin rebound effect observed in patients during the fir s t few hours extracorporeal circulation (E.C.C.) was studied in 13- Beagle dogs using a cold heparin and two radioactive (R.A.) heparins (S-35 et H-3). The E.C.C. was performed for 70ninutes (mn.) in all dogs, at a temperature of 37°C. in 4 dogs, 28°C. in 4 and 20°C in 5 (duration of hypothermia = 30 mn.). Results: 1) The injection of protam in reduced the plasmatic R.A. hepar in (factor 4-5) in a few minutes, then it remained constant. The residual heparin blood level was higher, after an E.C.C. in deep hypothermia than in normothermia. 2) The ratio of post E.C.C. urine R.A. to the total urine R.A. heparin (E.C.C. + post E.C.C.) was related to the temperature: 41% at 37°C., 30% at 28° and 25% at 20°C. 3) Three hours after the E.C.C., there was a selective storage of R.A.heparin in the liver. An hepar in rebound Was observed in all dogs after the protam in neutralisation; according to the results, this phenomena should be explained by a selective storage of the heparin during the E.C.C., or of the hparin-protamin complex after the E.C.C., in the liver, with a secondary release of the heparin.
